How about OPM has been not getting things right for a while, got hacked in 2015 where the Central Personnel Data File was put in jeopardy, ended up screwing up their entire security clearance background check work with huge backlogs, and still handles retirements on typewriters in an old coal mine. Restructuring may be beneficial.
