I don't understand enough of the American system to know how this scam works, but I don't get why there isn't a niche in the market for an insurance company that keeps down premiums by not allowing this kind of bullshit.

So, somehow the insurance company doesn't have a system in place to steer people from a ~3000 USD drug to two ~10 USD drugs? And they don't care, because they can just rack up premiums?
Is it that the drug company pays the difference in co-pay? Seems trivial for the insurance company to not allow the pharmacy to do that kind of shenanigans.

I think the problem is that the patient doesn't pay for the drugs directly. The insurance companies pay the pharmacies the exorbitant fees, then raise the cost of insurance just a little for everyone, yet both the pharmacies and insurance companies profit. Also, I don't think most people in the U.S. get to choose their insurance provider, their employer does it for them. In Canada most health care (but not drugs) are paid for through taxes, so no insurance companies get involved directly.
